Hours: 39 hours per week (inclusive of one sleep-in per week) – Monday to Friday with 1 in 4 weekends

Reports To: Registered Manager

Location: Crediton, Devon – fully on site

This is a genuinely exciting opportunity for an experienced care professional ready to take the next step into a clinical and operational leadership role within a small, values-driven service dedicated to supporting adults with Learning Disabilities and Autism.


Our client’s service has held a Good CQC rating for over 10 years and is now investing in its management structure to support continued growth and improvement. The organisation is looking for a trusted partner to lead the care team day to day, take ownership of clinical governance and quality assurance, and ultimately step into a more senior role as the organisation grows. About Our Client


Our client operates a specialist residential care home for adults with Learning Disabilities and Autism in Crediton, Devon. The service has been providing specialist care for over 35 years and is a well established, trusted home in the local community.


The service takes a careful, considered approach to admissions ensuring every new resident is the right fit for the community. The home has maintained a Good CQC rating for over a decade and is actively working toward higher good standards. Our client has recently invested in new care systems, an activities lead and an expanded staffing structure, and is now ready to bring in the right clinical and operational leader to support the next phase of development.


About the Role


The Care Management Lead is a newly created position designed to sit between the Registered Manager and the wider care team. This is a hands on, boots on the ground leadership role. The postholder will be on the floor working alongside the team, but will also have a designated supernumerary day each week for care planning, audits, rota management, DoLS assessments and operational administration. It is a role for someone who leads by example, is comfortable making decisions under pressure, and genuinely cares about the people they support.


The role incorporates quality assurance, compliance and clinical governance making it a substantive and varied position with real responsibility. In the absence of the Registered Manager, the Care Management Lead takes charge of the day-to-day running of the home.


Key Responsibilities


Care Management & Clinical Leadership

  • Take the lead role in overseeing the clinical management of residents’ conditions, improving quality of life through effective care, service and activity
  • Ensure each resident has an up-to-date, personalised care plan involving residents and relatives where possible, reviewed and evaluated regularly
  • Lead clinical responsibilities including infection control, medication management, tissue viability, wound care, nutritional requirements and moving and handling
  • Conduct and document care and risk assessments, ensuring areas of identified risk are appropriately monitored and acted upon
  • Act as the contact point for residents, families, visitors and external professionals


Team Leadership & Staff Development

  • Provide effective day-to-day leadership and supervision of the care team, acting as a role model for best practice at all times
  • Support the Registered Manager in delivering regular supervision and training to the team
  • Support new staff through induction programmes, training and competency assessment
  • Manage the staff rota on a weekly basis ensuring safe and effective cover across the service


Quality Assurance, Compliance & Administration

  • Complete required audits and compliance checks including staffing audits, HR file reviews, Legionella checks and other monthly monitoring tasks
  • Ensure high standards of record keeping in line with the Health and Social Care Act, CQC requirements and company policies
  • Ensure effective handover systems are maintained to facilitate accurate transfer of information between shifts
  • Understand and implement Mental Capacity Act and DoLS requirements, completing assessments as required


On-Call & Weekend Working

  • Participate in weekend working on a 1 in 4 rota this may be a full or half shift as required
  • Undertake one sleep-in per week (included within contracted hours)
  • Be available as a senior point of contact during shifts in the absence of the Registered Manager


Requirements


Essential:

  • Experience working with adults with Learning Disabilities and/or Autism in a residential or supported living setting
  • Experience of leading a shift and supervising a care team
  • Sound understanding of care planning processes with proven experience of writing and reviewing care plans
  • NVQ Level 3 in Health and Social Care or equivalent or demonstrable experience in lieu of formal qualification
  • Strong communication skills both written and verbal
  • Good organisational skills with the ability to plan, allocate and delegate work appropriately
  • Ability to work flexibly including sleep-ins and occasional weekend shifts
